House Call Visits

We started this practice with the sole purpose of giving our elderly communities a better access to their physician. This has remained the main tenant of our practice and will continue to be so.

If you are a patient who has difficulty walking, has limited access to transportation, or you enjoy the convenience of a primary care provider traveling to see you, then please reach out to our office for more information.

 

Our Services

  • An annual wellness exam is a yearly appointment with your primary care provider to review your medical history, reconcile medications, and update your care plan for preventive measures. If you're due for an annual wellness exam, schedule a visit with our providers—we will be happy to assist in maintaining your health and well-being.

  • Patients needing additional assistance at home, such as nursing or therapy, may be eligible for home health services, while those experiencing a decline in their condition may benefit from hospice care. Our providers are skilled in assessing patient needs and can refer to home health or hospice agencies if appropriate.

  • A Physician's Report Form 602 is a required document for every state licensed senior care facility in California. It is to be completed by a health care provider who will confirm a diagnosis, give TB clearance, and certify other healthcare needs and/or issues.

  • We've partnered up with leading imaging and lab companies to offer convenient, in-home diagnostic services for our patients. These services are offered through our trusted partners, and insurance coverage will need to be confirmed.

  • We understand that sometimes it’s very difficult to receive the care you need due to unforeseen events. We offer telehealth visits for our current patients in lieu of home visits.

  • Mental Capacity Letters are often required when obtaining a Power of Attorney or conservatorship over a family member. Our providers are experienced in assessing patients' cognitive capacity and their ability to manage their own medical and financial situation.

  • Everyone deserves to maintain control over their own medical care during times of serious illness. Advance Directing Planning and completion of POLST Forms (Physician's Orders For Life‑Sustaining Treatment) ensure patients' wishes are respected.

  • Our providers can assess and complete medical forms including FMLA forms, DMV Forms, Capacity Letter, Capacity Declaration Form For Conservatorship, IHSS Applications, and more. Please contact our office for any inquiries.
